No, You only need to send the Form 1040-V with your payment.  This is assuming that you have already filed the 2023 tax return, either by e-filing or printing and mailing the return.

No!  Not unless you are filing your tax return by mail.  If you e-filed your tax return, do not include anything other than a check with your 1040-V.
