What is the form that has to be printed out and mailed

there is a screen just before efiling that says Do Not Skip This Step! It said there was a form I had to Print Out and Mail to someone (IRS?) But every time I clicked on the Button, all I got was an Error. What Form was I supposed to print out and mail?
