Yes, since you have some charitable contributions that require a little extra, you will need to send your documentation with the DR 1778. Failure to submit the form can cause denial of credit.

I have the same issue. I have never had to mail in anything but this year, it printed out the DR 1778 along with my return. The problem is that I dont have receipts of the donations I made. In this case, does not sending in the DR 1778 void my return? Would it be best to amend my tax return to just take the charitable donation out?
This excerpt from the instructions may be helpful (this is from line 12 of the DR 0104AD Instructions:(
For claims greater than $5,000, submit the receipts you received at the time of donation. For in-kind donations, submit an itemized list of the donated items and their fair market value. Submit using Revenue Online or include with your paper return. Do not send receipts of items that were purchased for donation.
At the very least, include the list of items with clear names.
Also, you might visit this page in Revenue Online, which claims that you can file the DR 1778 online. You may want to go here first, in case it makes more sense.
