What do I mail in with an amended 1120S?

I read this on the IRS website but can't find the form.  Where can I find it?

You can look up where to mail an amended 1120S at Where To File.

The passage you have is from Amended and superseding corporate returns.

HINT: If you are going to amend or supersede your return, you can use the Save as... from the File menu- use AMENDED so that you will have the original file and the amended.

Yes, but my questions is where do I get the form to mail in with the amended return?

There is no new form to mail in with an 1120-S.  There is just a tick box on the first page of the 1120-S that says "Amended Return".  You check that box and then make the changes that you need to.  If you are using TurboTax as soon as you say that you are amending the return TurboTax ticks that box for you.
