What kind of 1095? There are three different types: 1095-A, 1095-B, and 1095-C.
If you had insurance from the Marketplace (Obamacare) you need your 1095-A. You should be able to get another copy on the Marketplace web site, or call the Marketplace and ask them to send you another copy.
If you did not have Marketplace insurance, you would have gotten a 1095-B or 1095-C from your employer or insurance company. You don't need that form for your tax return, so don't worry about it. You don't have to do anything.
