Computers produce many of the letters the IRS sends out without a human involved.
Your Form 1040-V and estimated payment vouchers were probably sent out because some missing info told the computer you might owe money.
A lot of things may have happened. The IRS may have lost your return. If you e-filed, maybe some of the data was corrupted. The IRS may still be processing W-2 information from your employer so it was not able to verify the accuracy of your wages or withholding.
The IRS is still backed up from Covid-19 lockdowns, social distancing and having to send out three stimulus checks plus advance child payments in the past two years. Three weeks is probably too short a processing time.

That is probably right. Maybe some of my info was lost in the e-file process. In the 12-c they did want verification of income so maybe so info didn't transmit. As for the 1040-v and vouchers, no letter or any form was with it showing an owed amount. If it would have been I would know 100% that it was a mistake.
