W-7 form for spouse

My spouse does not have a SSN number, how do I submit my tax return along with the W-7 form as instructed in the IRS website?

Use the following "dummy" ITIN's in place of the social security number:

999-88-9999
999-88-9998
999-88-9997
999-88-9996
999-88-9995

You will have to mail in the W-7 form with your income tax return. Please see the instructions below:
