It's quite possible that the IRS discovered the name and identifying number mismatch when you submitted the form, and has already corrected the issue without notifying you.
However, go ahead and file an amended Form 3520, specifying the tax year you're amending. You don't have to amend your entire Form 1040-series return, but you might want to attach a letter explaining your situation.
