First see if Costco will take your product back. If not, you could try using this Online request form.

Since you didn't purchase it from TurboTax, you will see there that you need to mail the CD with a copy of the receipt. Costco should be able to print you out a new receipt, since they track your purchase history through your membership card.
Or phone TurboTax Support and see if you can work something out with them.

Where the form asks what product you are using, be sure in the dropdown menu to indicate "CD/download" and the proper operating system, then indicate which Edition.
