trying to enter a non 1099-b

I need to enter a non 1099-b for a worthless stock.  When I get to the skip import it brings up the 5 types. There doesn't seem to be any way to enter a non 1099-b.  When I select "Stocks, Bonds, Mutual Funds" it still doesn't allow for a manual entry.  Thanks.

 click on skip import. pretend you got a 1099-B title it anything you want then continue. answer following questions. select entry one-by-one. select long-term - did not receive 1099-B 

.then on a page check the worthless box/ should appear on line 10 of schedule D.
