Trouble Getting to Form 4582 Option

I'm working with TurboTax online (free edition), trying to include a Form 4582 with my return because I did not get a W-2 from an employer. TurboTax help (https://ttlc.intuit.com/community/accessing/help/how-do-i-file-a-substitute-w-2-using-form-4852/00/2...) says to check the "Substitute Form" box on the "Do any of these uncommon situations apply to this W-2?" screen, but there is no Substitute Form box on that screen. Instead, I see only:

* Worked outside the U.S.
* Paid family leave
* Unreported tips
* Religious employment
* Foreign or international organizations
* Pension plan
* Corrected W-2
* Nonstandard W-2
* Inmate or Halfway House
* None of the above

How do I get to the Form 4582 option through TurboTax?

the form is 4852 maybe that's the problem
