The corrected W2-c I received lists the Wage Box 1 blank under both previously reported and correct info. Do I leave these blank or use info from original W2?

You would use the W-2C, and they would be blank.

Is there any dollar amounts on the rest of the W-2?  If not, then the W-2 does not need to be reported.
