Tax Repayment Plan

This year I owed taxes for my employment as a contractor. I know you must mail in the first payment via a specific form. I remember seeing the form at the tail-end of my turbotax filing process, but can no longer find the form. Thoughts on where I can access this? (it was personalized based on my location I believe)

You can find that when you download your state and federal return using the steps below.

1. Login to your return

2. On the left hand side you will see the status of your returns under that is a download pdf option.

( Please note: That voucher will be in your state return/ federal- whichever you owe money to.)
