received a state refund from 2019 in 2021. I itemized deductions so I have to report under other state/local tax issues. Question: for the "total all your payments and withholding" block, do I go back to 2019 for that?

No. You can find most of this info on your 2020 state tax return.
For the question Total of all your payments and withholding, you must add up any payments you made to your state or locality, including:
- Amounts withheld on your W-2, W-2G, and 1099
- Estimated tax payments for 2020 (including any paid in 2021 for 2020 - it's common for the fourth quarterly payment to be made in January)
- Overpayment from 2019 applied toward your 2020 return
- Payments made with an extension of time to file
- Refundable credits
thank you.
