Short-term disability

My short-term disability payments do not show up on my W-2 form. The short-term disability insurance is paid for solely by my employer, so I know it's taxable. Where do I get the information to put on my tax forms?

Amounts you receive from your employer while you're sick or injured are part of your salary or wages. It is included in box 1 on your W-2.
