RRB-1099-R question on missing box number

I am first time user with Turbo Tax.  On entering the RRB-1099-R (green) directions say Box 12 Medicare Premium Total.

There is no box 12 on the form.

Box 11 on the blue form (RRB-1099) says Medicare Premium Total.

Do I use the MPT in box 11 for the question that says box 12?

Thank you
