The control number is not required. You can leave it blank or enter 1234.

Are you trying to import the W-2? The W-2 for short-term disability might not be available for importing. Just enter it manually. That will be much faster and easier than trying to get the import to work. Importing a W-2 is a nice convenience, but it only takes a minute or two to enter a W-2 manually.
