I have downloaded turbotax amend 2018. I have been fighting SSA for almost 3 years to get me an amended 2018 SSA 1099. Received it today. On turbotax Amend I do not see a box 4 on the SSA 1099 screen that I can check to show the money was repaid to SSA in 2018. Please advise.

On your tax return you enter only the amount from box 5 of Form SSA-1099. That would be the net amount of benefits that you received, after subtracting the repayment. On the amended SSA-1099 box 5 should be less than box 5 on the original SSA-1099 by the amount that you repaid. When you change the amount in box 5 to the reduced amount, TurboTax will recalculate your tax to adjust for the amount that you repaid.
Thank you very much. Since I paid it all back I would put $0 in the box and the IRS would confirm with my amended 1099?

If box 5 on your amended SSA-1099 is zero (or blank), then yes, you should enter zero in box 5 in TurboTax when you make the changes for your amended tax return.
In your amended return you will have to enter an explanation of the changes. You should say something like "Received corrected Form SSA-1099 with box 5, Net Benefits, changed to zero."
You will have to file your amended return by mail. Attach a copy (not the original) of the amended SSA-1099 to the amended return.
Mail the amended return by certified mail with a return receipt so that you get tracking and confirmation of delivery. Because the IRS is severely backlogged, it will take a very long time for your amended return to be processed.
