1099R form missing Payer address?

I received a 1099R form for early withdrawal out of my retirement account. When filing Turbotax is asking for each Payer's address. My 1099R forms that I received does not include this information. But Turbotax will not let me complete filing because of this. Is there a way for me to bypass this and if not how can I get this information?

You can't get past the error for ''no address'' on the 1099-R.  There should be a phone number on the 1099-R so that you can contact the issuer and get the address.  The address has to be entered or you won't be able to get through the File section and submit your returns.
