I am helping my mom with her taxes using Turbo Tax. She has a 1099-R that says the payer is from the state of Indiana, which is where she lived 10 years ago. My mom lives in Florida. Does this mean she will have to fill out a state return for Indiana just for this one 1099-R? They also had her old Indiana address on the 1099-R but she called them to change the address.

Did she work for the state? The payer is irreverent. The state only matters if there was state tax withheld in box 14 and the state ID in box 15.
Box 14 shows zero taxes withheld. Box 15 says Indiana. Does that change anything from what you said before? Thank you.

Then the IRA custodian sent that the money to the state of Indiana. She needs file an Indiana non-resident tax return to recover that money. She should be sure that the account custodian gets her new address and that she is no longer a resident so they stop withholding Indiana tax.
