1099-B with missing or empty boxes

I have a 1099B form, It statesSale Category: Code X- Unknown Holding Period.  

 

1b) Acquired Date is empty on the form Acquired before 2011

1e) Cost basis Is not given

Box 2) which is Long-term or Short-term is empty, not given

 

Do I just enter my values for the missing data? Do I need to enter some type of adjustment?  As I tried entering a correction under (I'll enter additional info on my own) button, but it says, I can not fill box 2 correction for a "no entry".

Do I just enter my values for the missing data? Yes--as if it were on the 1099-B.
