I assume the brokerage sold fraction share of stock to pay their own yearly fees.

If i do, need to fill out then what should i put in "Box 1e - Cost or other basis".

Its empty on the 1099-B form mutual fund company sent me.

Yes, you should report the sale- you can enter a zero basis.
