Question regarding filling form 8843

I have a question regarding filling the form 8843.

I filled the part 1 of the form 8843.

Part III doesn't apply to me anymore as I wasn't a student last year. In 2020, I worked on OPT for first 9 months and then on the H1-B for the last 3 months.

Part II, Part IV, Part V don't apply to me at all.

My question is should I still fill the part III as I was on OPT for first 9 months last year.

Yes, you would still need to complete part III. Although you have started working under OPT, you are still under the F student -visa. OPT is a special program and part of your F-visa.

How would I answer the question below while filling the part III:

"Enter the name, address, and telephone number of the academic institution you attended during 2020"

As I didn't attend the academic institution durning 2020. Should I still mention the details about my institution here ?
