Program wants additional information when I try and e-file?

I completed my returns and tried to e-file and the system told me it needed some more info that they had not yet requested, so then I got a pop-up that seems to be a form 4852 (missing W-2) and there is really no instruction on how to fill this out.  Also, I am not missing a W-2. So where it asks "where did you get your W-2 info, I answered "My W-2" Then it asks me how many times I tried to obtain the W-2 from my employer, and I answered that I have my W-2. 

But then I hit continue and I get the same screen again - over and over. I cannot tell what am I doing wrong.

I would go back & make sure all the info from your W2 is entered correctly.
