Payer's address on form 4852 when the bank is in Puerto Rico

Filling form 4852 substitute to missing 1099-R on IRA distribution from Bank in Puerto Rico. Only accepts the address as foreign using ¨Other Countries¨ in the country selection and further selecting PR in a subsequent list (posted as Province). However when navigating out of the form, TurboTax asks to review the address (postal code and country) because supposedly includes elements of both a domestic address and a foreign address. Change all the possible combinations and the review request does not go away. Can I use Turbo Tax to file with this form or do i have to file differently. Last year it accepted the address and efile without problems.
