On my W-2 form box 15 is required (the Employers State ID Number) but it is empty. What do I do?

You can use the federal ID number that is listed in box "b" if there is no state ID number listed on your W2 in box "15".

You can also call your employer's HR dept. and ask after you have ID'ed yourself as their employee, they should share it.
