I have my own health insurance through my employer and my wife and kids are on Medicaid after i filed my wife got a letter showing there coverage do i need to amend

its says on the back of the form this is for you or any person claiming you as a dependent my employer didnt issue me one of these ? so i dont know if i need to file my wifes on my taxes i claim her my kids and i file head of house hold

No, if you are married then you cannot file head of household. You can use married filing jointly or married filing separately only. See the FAQ below. https://ttlc.intuit.com/replies/3302320 

Also, not all 1095 forms get reported on your tax return. Only Form 1095-A must be entered into TurboTax. If you indicated on your tax return that you had insurance all year, and it was not Obamacare, then no, you do not need to amend.
