Is an injured spouse form needed if I'm filing as head of household?
I have always filed an injured spouse form when filing as married filing jointly. This past year, my husband and I separated and I am filing as head of household. I still had to list his SSN on my return, so I am concerned that my refund will still go towards his government debt. Do I need to file an injured spouse form this year?

No, if you are filing separately then you will not have to complete an Injured Spouse form.
