NH resident. In addition to my usual income (self employed) I earned $500 from a client in Maine, for work in Maine. How do I post that? (no 1099 for this, it was cash). Included in gross income. Do I need a Maine tax form?

It depends on how many days you were in Maine for business, but $500 is less than the $3,000 filing threshold for Maine.  If you were also in Maine for 12 days or less, you do not meet the minimum taxability threshold and do not have any tax liability.
