Middle name and first name are swapped on w-2, is that OK??

I go by my middle name, so my previous employer put that as my first name and then the first initial of my *actual first name, as my middle name. Is this fine?

Yes, this will be fine with the IRS. They are matching your last name and Social Security Number. As long as those items are entered correctly, there will not be a problem at all.
