Married filing jointly, statement as per IRS publication 519

I and my spouse are filing the joint tax return. We are attaching a statement as per IRS publication 519, page 9 for Nonresident Spouse Treated as a Resident  Publication 519 US Tax Guide for Aliens 

Is there a specific format we have to follow?

Take the language in that "How to Make the Choice" section and substitute your names for the "one spouse" and "other spouse" and both your names for "you".  For "each spouse" in the second "bullet" list the name, address, and SSN or ITIN for that spouse.

For example:

We declare that John Doe was a nonresident alien and Jane Doe was a U.S. Citizen on the last day of 2019, and that John Doe and Jane Doe choose to be treated as U.S. residents for the entire tax year.

The name, address, and identification number of John Doe is:

[put info here]

the name address, and identification number of Jane Doe is:

[put info here]
