Mailing form 8822

I needed to complete Form 8822 for a change of residential address in 2016.  I downloaded, completed, and printed the form from the TurboTax site.  I believe a second page was supposed to print containing mailing details.  I understand that the tax return and Form 8822 must be mailed separately.  I never received mailing details and have been unsuccessful finding this information on the site or online in general.  I live in Wasilla, AK and I need to know which is the appropriate IRS address to mail these details.  Thank you for your assistance.

We're sorry to hear that the instructions didn't print out for Form 8822 (Change of Address with IRS).  You are correct that this particular document should be sent separately from your tax return.  That said, your mailing address for sending Form 8822 to the IRS (from Alaska) is:
