I had an accountant complete the K1 report for our newly formed LLC.  I am doing my personal return and it calls for a 1099 from my K1 report.  I do not see this report.  Is it on the K-1 form 1065?

No, a k-1 form is an entity alone, think of it as a w2 for your business. It is full of information and is not related to another form.
