Is all disability income reported

my wife did not receive a 1099 for her disability. what can I do. do I need to report this income

No, if you or your spouse paid the disability income premiums out of pocket, the disability income is not taxable or reportable.

Disability income is considered taxable income if the premiums for the disability insurance were paid by your employer, or if you paid for the premiums with pre-tax dollars on your paycheck (If you are not sure, check with your HR department).
