Insurance premiums treated as an HSA account

Last year I was briefly enrolled in an employer-provided life insurance program. This account was cancelled after about two months and the money refunded.

 

Now that I've received my W2, the premiums paid into that account are showing up here under code W, which appears to be a code for HSA contributions. I never had an actual HSA account last year, nor do I have a high-deductible health plan, but the fact this code is on here now requires me to fill out a 8889 form which I cannot do (no HDHP plan).

 

I've checked with my employer and they verified the code to be correct on my W2. What should be done here?

code w in box 12 is specific for contributions under a cafeteria plan to an HSA.  ask the employer to provide documentation that the money was sent to the trustee/custodian. also, ask for the name of the custodian and phone number so you can call and get the required tax forms you need for filing.   you may eventually need the help of a tax pro and possibly need to contact the IRS if the W-2 is incorrect and they refuse to correct it.
