I have filled out a Installment Agreement Request to Initially Pay $1000.00 then $250.00 monthly.
However when I printed all my forms, there are PAYMENT VOUCHERS to be mailed in with different amounts. (4 @ $613.00). What do I do?

The Form 1040-ES are not payment vouchers. The are for paying 2020 estimated taxes based on your tax data from 2019. They are for your use only and totally optional.
The Form 1040-V is a payment voucher for the 2019 taxes owed on the 2019 tax return.

If you applied for an ACH withdrawal for all payments, to include the initial payment, then you do not need to mail the Form 1040-V and payment with that form.
