The form has already been added manually in forms view and it appears that the adjustment details (amounts, years, and properties) are entered at the end of it when I print out a copy of the form using pdf.
I noticed that on line 14a thru 14d, detailed explanations have to be provided. Does the information above count or a separate statement need to be attached (if so, how)? Does anything else need to be attached when e-filing?
Also, according to the IRS instructions another copy needs to be mailed as well. What needs to be included with the 3115 (copy of 1040, 8853, detailed statement, etc)?

Unfortunately, TurboTax does not support this form for efiling. TurboTax allows you to complete and print this form in Premier. You will have to mail your federal return in.

Automatic change requests. Except if instructed differently, you must file Form 3115 under the automatic change procedures in duplicate as follows.
• Attach the original Form 3115 to the filer's timely filed (including extensions) federal income tax return for the year of change. The original Form 3115 attachment does not need to be signed.
• File a copy of the signed Form 3115 to the address provided in the address chart on this page, no earlier than the first day of the year of change and no later than the date the original is filed with the federal income tax return for the year of change. This signed Form 3115 may be a photocopy. For more on the signature requirement, see the Name(s) and Signature(s)section on page 3
