When it asks if my company has a name, do I put the name of the company that pays me or is that for if I have a different name for myself individually?

As an independent contractor, the name for your business is usually your real name, unless your business has a name. Don't enter the name of the company that pays you. They are your customer and you are the contractor, in this situation.
