On my W2 box 1 is $0.00. (ihss payments)

Do I add and fill out a W2 under Wages & Income in TurboTax if the amount in box 1 $0?

Do I still attach a copy of my W2 to my federal return?

If box 1 is zero, you don't have to include that W2 on your tax return.

You do not have to file the Form W-2 with your tax return, keep it for your records.
