If I am a dependent under my parents employers insurance, when filing state, how can I prove I have insurance? I dont have 1099-HC I have 1095-C. Can I utilize their EIN?

Currently in the step of proving to my state that I met MCC. It asks me to put insurances information but the boxes ask for info that is provided on 1099-HC form , I only have 1095-C. The only EIN number on it is my parent.

As a dependent covered by your parents’ company insurance, you do not need to receive a Form MA 1099-HC directly. Carriers may send separate Forms MA 1099-HC to dependents if they choose to do so. However, if you need to file a Schedule HC, you have two options:

1. Obtain Information from the Subscriber’s Form MA 1099-HC: You can gather the necessary details from your parents’ Form MA 1099-HC.

2. Request a Separate Form MA 1099-HC from the Carrier: If needed, you can ask the carrier to provide you with a separate Form MA 1099-HC.

Thanks for your response, and for explaining how to obtain the form.  I am in the step of proving I have insurance and it wants me to input information from a 1099-HC. As I had mentioned, I only have 1095-C, it says that if I dont have the 1099-HC form to just input the insurance card info. When doing so it keeps rejecting the information because it wants an EIN. Can I utilize my parents EIN since its the only one on the form? I am unable to get the 1099-HC because my parents employers insurance is from another state.
