I work in home health care and hospice doing massage therapy. This site is asking me for an address of my business. I work in peoples homes and health care facilities. What am I supposed to do?

Please enter your home address as your business address when you are self -employed and do not have a specific business location.  The same applies to your name and business ID. The business name is your name and the federal ID is simply your social security number.
