You do not enter a Form 1095-C on a tax return.  On the federal tax return you are not required to answer whether or not you have health care insurance.

Yes, Form 1095-C is used to report employer insurance coverage. This is considered private insurance. 

 

You don't have to report Form 1095-C on your federal return, but you may need to report it on your state return, if you are filing a state return with a penalty for not having insurance coverage. 

 

For example, if you are a Massachusetts resident, you need to report that you have private insurance coverage on your state return. You will also need some information from Form 1099-HC to complete your state return.
