I have a W2 from other state

I have a W2 from Washington, lived there for 3months with my uncle and his family but came back to Arkansas, where I am a permanent resident, so the question is do I include the Washington's W2 on my return?

Yes you do.  Your state of legal residence can tax you on all your income, regardless of where you earned it.
