You do not have to file that form.

What if I receive the form but didn't take the insurance and checked no on my taxes?

bsch4477 is right, the new 1095-C Form, Employer-Provided Health Insurance Offer and Coverage, does not need to be filed with your tax return.
When asked the question in TurboTax, "Did you have health insurance coverage in 2015", simply select “Yes”.
Be sure to review the form for accuracy and keep it with your 2015 tax files.

On taxes it ask if I have a 1095A, I have a 1095 C. so where do I say yes I had insurance for 12 months
