You need your actual W-2 from Walmart.   Do you still work there?   Have you asked someone in the Human Resources office how to get your W-2?  If you can get a copy of your W-2 you can type in the information yourself or maybe you can import it but in order to import it, you need the amount from box 1 and the EIN.   Call the Walmart, text them, email them, stop in to the manager's office if necessary to get your W-2.
