3 months under insurance no selection for medicare and I am getting penalized ?

You would check the box that you were covered for the months you had coverage through medicare.  If you were covered through a different plan the other months, check the boxes for those months.  Or simply say you had coverage all year if you had Medicare and other coverage for the entire year.
