How to handle 1099-B for an acquisition

My company was acquired and I exercised my options so they could be converted to the new parent's equity. I got a 1099-B. But it doesn't have line item sales listed, there are actually just two 1099-Bs with almost no info on them except the company name, a date sold, and a value in 'Proceeds.' No info for cost basis or date acquired, boxes 8-11 are blank. How do I file this?
