how to add third party designee to form 1040-SR

It depends on which version you are using.  

If you are using the online version, you will need to print your return to add it and then mail your return in.  You would not be able to e-file and add a third party designee in the online version.

In the desktop version, you will just need to switch to forms mode.  Once in forms go to the Infor Worksheet and put a check  mark in the yes box for Do you want to allow another person to discuss this return with the IRS and then enter their name and phone number and any 5 numbers.  Then you can switch back to Step-by Step to e-file your return.
