If you are mailing a check for the Louisiana taxes due, include the R-2868-V payment voucher along with your payment and mail to:

Louisiana Department of Revenue 

P.O. Box 751 

Baton Rouge, LA 70821-0751

The payment voucher should be included with your printed tax return, you can also print the voucher here http://revenue.louisiana.gov/TaxForms/2868V(1_18)F.pdf.
