How do I report my taxable scholarship money on my Grand Rapids City return?

I am currently filling out my Grand Rapids city tax return and I'm unsure how to report my scholarship money. Turbotax automatically took care of counting scholarship money in excess of tuition as income for my Federal and Michigan returns. However, as part of the city return I am being asked to fill in "Employer ID" when none exists and my "Actual Work Location." The scholarship money is not excludable based on the note in TurboTax. How do I go about reporting taxable scholarship money on my city return?

How do I report my taxable scholarship money on my Grand Rapids City return?

Use the EIN or "Employer ID" for your school. It should be listed on your 1098-T or get it from the school office. 

Actual work location would also be the school address.
